Custom Forms

In this article, we'll explain how documents and forms can be used in Funeral Buddy. 
There are two ways of implementing the use of custom PDF forms in Funeral Buddy: 
1. Via a Custom Task Template using the field "Upload Document Name". As soon as an RDD file is uploaded here, the task will require a document uploaded as part of the task.
There is also the Document Form Determined By drop-down, which determines how the document is generated. The option for this field is called "CUSTOM DOCUMENT PROVIDED BY USER".
When a form is generated for such a document the system will try to find the User Defined Document with the name specified in the task template. All the corresponding tags/fields will be filled in automatically based on the data collected in the case.
One needs to be logged in as an Administrator to access the above menu, and if using the cloud version, you will need to engage our staff to generate the custom RDD file representing your custom form. For the users with the purchased version, you can use the report designer to create your form. Once created, uploaded and associated with a custom task, at the time specified in the task template, there will be a new task, and the Uploads area of the case will have an entry. There will be no Upload button for this record - just the Generate Form button. The GenerateForm button will populate the document, upload it automatically and display the result.
2. Via a custom document associated with the interview process. For this, you must also be logged in as an Administrator and navigate to Shortcuts>Manage>Document Forms> Add New Document.
For the form type, you need to select one of the following options as applicable:
  • Application for Burial
  • Application for Cremation
  • Digging Requisition
  • Authority to release the body(Coroner)
  • Authority to release the body(Medical Superintendent)
  • Permission to cremate
  • Authority to collect ashes

Select the corresponding State the form will apply for , and the type needs to be "Report". Select the RDD file for the form and upload it. After saving this, the next case where this form can apply will feature the display / generate option during the interview process or in the family link as applicable. This form can also collect signatures, unlike the custom task forms.
